ADAPTIVE EQUIPMENT PROCESS

STEP 1: ELIGIBILITY

Children are eligible for equipment based on the following criteria:

  • 17 years of age or under

  • Requests equipment that qualifies as a valid health need

  • Lives in the Greater St. Louis Region

  • Referred by a social worker, case manager, etc.

  • Requests up to $1000 for Adaptive Equipment

STEP 2: APPLICATION

Parents or legal guardians complete the equipment application below.

STEP 3: DOCUMENTS

To fully evaluate the application, email or mail the following required documents to The Arya Foundation: (Please do NOT fax the documents)

  • Letter from the physician stating the necessity of the equipment

  • Invoice from the provider or online URL

  • Child’s Photo & Story

STEP 4: REVIEW

Within 30 days of the application being submitted and the required documents being received, an application decision will be sent by email. If approved:

  • A check payable to the provider will be mailed to the family or the equipment will be shipped to the family, depending on the equipment

  • The child’s story and photo may be posted and used for The Arya Foundation’s advertising if a media release form is signed
