Adaptive Equipment Process
ADAPTIVE EQUIPMENT APPLICATIONS SUBMITTED BETWEEN OCTOBER 1 - DECEMBER 31 MAY TAKE UP TO 90 DAYS TO PROCESS DUE TO THE NUMBER OF HOLIDAY ADAPTIVE TOY APPLICATIONS
Step 1: Eligibility
Children are eligible for equipment based on the following criteria:
17 years of age or under
Requests equipment that qualifies as a valid health need
Lives in the Greater St. Louis Region
Referred by a social worker, case manager, etc.
Requests up to $1000 for Adaptive Equipment
Step 2: Equipment Application
Parents or legal guardians complete the equipment application below.

Step 3: Submit Documents
To fully evaluate the application, email or mail the following required documents to The Arya Foundation: (Send the documents via email, we don’t have any FAX.)
Letter from the physician stating the necessity of the equipment
Invoice from the provider or online URL
Child’s Photo & Story

Step 4: Application Review
Within 30 days of the application being submitted and the required documents being received, an application decision will be sent by email. If approved:
A check payable to the provider will be mailed to the family or the equipment will be shipped to the family, depending on the equipment
The child’s story and photo may be posted and used for The Arya Foundation’s advertising if a media release form is signed
